What is the Arizona Living Well Institute?

The Arizona Living Well Institute is a public-private partnership encompassing all sectors of the healthcare community. The mission of the Living Well Institute is to advance evidence-based programs for Arizona communities through structured communication, multi-level coordination and facilitator coaching. As part of the health education and wellness programs at Empowerment Systems, Inc., the Living Well Institute is committed to developing partnerships with like minded organizations to contribute to the overall well-being of individuals, families and communities.

Programs Available


The Living Well Institute is partnered with the Arizona Living Well Program at the Arizona Department of Health Services to deliver the Healthy Living: Self-Management of Chronic Conditions workshops throughout the state of Arizona. Healthy Living is a six week chronic disease self-management program (CDSMP) developed by Stanford Patient Education Research Center. The Living Well Institute facilitates communication, coordination, and coaching for organizations and individuals who are interested.
Additionally, the Living Well Institute provides technical assistance and resource support for Arthritis Self-Management Program, Tomando Control de su Salud (Spanish version CDSMP), Programa de Manejo Personal de la Diabetes (Spanish Diabetes SMP), and Better Choices, Better Health® (online version CDSMP), developed by Stanford Patient Education Research Center.

Partnership DevelopmentAZLWI Healthy Living

AZLWI was launched in April 2010 with grant funds from St. Luke’s Health Initiatives and the Arizona Department of Health Services through a grant from the Administration on Aging.  Currently, the AZLWI is funded through multiple avenues including the Arizona Department of Health Services, Virginia G. Piper Charitable Trust, an AmeriCorps grant with St. Luke’s Health Initiatives, and a recently awarded Dignity Health Grant with the National Kidney Foundation of Arizona and the Cardio Renal Society of America. To successfully deliver Healthy Living workshops throughout Arizona, we recognize the need to work collaboratively with community-based organizations, healthcare systems, community health centers, senior centers, tribal and minority leaders, parks and recreation centers, employers, and providers. It will take the volunteers, staff, and providers within these organizations to help spread the word about the benefits of Healthy Living. Our partnerships are developed through regular communication between organizations that is facilitated by regional lead mentors and sub-regional coordinators.  The AZLWI continues to be partnered with the Arizona Living Well Program, Bureau of Tobacco and Chronic Disease, and Division of Behavioral Health Services at the Arizona Department of Health Services to provide coordination, communication and coaching for the Healthy Living Workshops.
