The Administration on Aging recently hosted a webinar on successful collaborations between Federally Qualified Health Centers (FQHCs) and the Aging network in two states: Missouri and New Jersey. The Arizona Living Well Institute, Arizona Department of Health Services, Area Agencies on Aging and other community-based organizations have partnered with several FQHCs in Arizona to deliver and sustain a system of Stanford Chronic Disease Self Management workshops across Arizona.
Why is this important for Arizona?
- Approximately one-third of FQHCs report that older adults comprise up to 35% of their patient pool.
- Collaborative efforts between FQHCs and other healthcare providers reduce costly duplication of services and overt competition.
- Collaboration projects foster increased access to quality healthcare and continuity of care to vulnerable populations.
